position is responsible for providing skill and support development
to consumers in their living, learning, working, and social
environment within the framework of a service plan. Essential
Functions Coordinate services and initial consumer contact with
Case Manager at the anchor agencies. Provide skill development and
support in a one-to-one relationship. Assist consumers in the
development and implementation of long and short-term
rehabilitation goals. Develop specific, achievable,
behavioral-based and objective goals that address the skill areas
the consumer has identified. Assist in the development of
strategies to enable consumers to meet their rehabilitation goals.
Provide direct care skills training in activities of daily living.
Consistently assess, teach, evaluate and supervise consumers'
skills in their living, learning, working, and social environments.
Prepare service plans, progress notes, case summaries and other
related paperwork in a timely manner. Organize schedule in order to
maximize face to face contacts. Complete assignments independently
and within specified time frames. Provide accurate and detailed
tracking of service units. Demonstrate the ability to take
direction. Follow instructions accurately and thoroughly. Adhere to
supervisor’s requests. Show initiative and demonstrate good
judgment. Demonstrate the ability to make sound judgments regarding
the welfare of consumers. Work flexible schedule to accommodate
program and consumer needs. Demonstrate knowledge of basic
principles and methods of rehabilitation. Use appropriate
communication channels. Display motivation and interest in working
creatively with people who have mental illness. Perform the job
within the framework of agency, site and professional standards.
Participate in staff meetings and training programs. Participate in
consumer meetings with other service providers. Participate in
supervision on an on-going basis. Attend and participate in agency
events. Adhere to agency/program policies and procedures. Project a
positive attitude and image to staff, consumers, family members and
outside contacts. Participate in achievement of agency/program
objectives, goals and mission. Cooperate with other staff and
exhibit positive attitude toward position and agency mission.
Follow expected dress code. Transport consumers as
necessary/appropriate. Respond appropriately to emergency
situations. Provide service to a caseload of ten consumers.
Performs job in a safe manner. Reports/addresses safety concerns in
a timely manner. Performs other duties or special projects as
required or as assigned. Other Functions Attends and participates
in regular supervision. Responds appropriately to emergency
situations. Performs job within framework of agency, site, and
professional standards. Participates in staff meetings and training
programs. Attends and participates in agency events. Adheres to
agency/site policies and procedures including HIPAA requirements.
Cooperates and coordinates with Regional Director on all
operational issues. Projects a positive attitude and image to
staff, participants, family members, and outside contacts. Uses
appropriate communication channels. Completes assignments
independently and within specified time frames. Participates in
achievement of agency objectives, goals, and mission. Displays
motivation and interest in working creatively with people who have
mental illness/disabilities. Demonstrates the ability to make sound
judgements regarding the welfare of people served. Displays ability
to take direction. Demonstrates knowledge of basic principles and
methods of clinical psychology and psychiatric rehabilitation.
Follows agency dress code. Adheres to supervisor's requests.
Cooperates with other staff and exhibit positive attitude toward
position and agency. Contributes to the growth and development of
services. Shows initiative and demonstrate good judgement. Follows
instructions accurately and thoroughly. Works as scheduled to

safety concerns in a timely manner. Requirements: Qualifications
Required Education: High School Diploma or GED Preferred Education:
Bachelor's Degree Field of study: General Studies, Social Sciences,
or Mental Health Work Experience: 6 years’ experience with HS
Diploma or 2 years with Bachelor’s Degree Certifications, Licenses,
Registrations Required: Valid Driver’s License Knowledge, Skills
and Abilities Knowledge of psychotropic medications preferred.
Knowledge of basic principles and methods of rehabilitation
required. Motivation, interest and ability to work empathetically
with people who have mental illness required. Ability to work
independently and effectively handle multiple tasks required.
Ability to work as a leader required. Ability to make sound
judgments regarding the health/welfare of consumers required.
Ability to handle confidential information required. Ability and
willingness to drive agency car/van required. Knowledge of
Microsoft Office products. Strong collaborating skills, and an
ability to work effectively both independently and as part of a
team. Comfortable in a fast-paced environment with frequent
deadlines; high energy, self-motivated and proactive. Excellent
communication and interpersonal skills. Ability to multitask and be
flexible with regard to workload and assignments. Ability to

place objects. R Feeling – required to discriminate between varying
textures. R Grasping – required to pick up objects with fingers. R
Substantial Movements – required to perform substantial movement
(motions) of the wrists, hands, and/or fingers. O Eye, Hand, Foot
Coordination – required to coordinate the eyes, hands, feet with
each other in response to visual stimuli. O Motor Coordination
Skills – required to coordinate eyes, hands, and fingers rapidly
and accurately and handle precise movements. R Color Determination
– required to identify colors through vision. R Near Acuity –
required close, clear vision with or without correction. O Depth
Perception – required to distinguish depth. O NOTE: At the

experience and/or education may be substituted in lieu of the
requirements specified above under Education and Experience.
