Job Description:
Engage with individuals on the street who appear homeless.

Maintain an active registry of chronic and non-chronic homeless
clients.
Conduct vulnerability assessments to identify client needs.
Attend clinical supervision sessions and relevant training.
Develop and maintain client records, databases, and necessary
reports.
Communicate with team members through progress notes, reports, and
meetings.
Liaise with service providers on behalf of clients.
Collaborate with agencies such as Parks Department, NYPD, DSNY, and
MTA.
Participate in Code Blue/Red initiatives during extreme weather
events.
Monitor encampments to ensure safety and connect individuals to
services.
Perform other related duties as assigned.
