MLTC Intake Nurse – RN Location: Brooklyn, NY (Hybrid) Language
Skills: Chinese or Spanish a strong plus Salary: $95,000 – $105,000

Term Care (MLTC) program serving the diverse communities of
Brooklyn is seeking a Registered Nurse to join its Intake team.
This is a hybrid role requiring field-based assessments and
in-office collaboration. The MLTC Intake RN plays a vital role in
evaluating potential new members for program eligibility, educating
them and their families on available services, and supporting a
smooth transition into long-term care. Bilingual language skills in
Chinese or Spanish are highly preferred due to the populations
served. Key Responsibilities: Conduct in-person assessments using
the UAS-NY tool to determine program eligibility and safety for
potential members. Educate prospective members and families about
the MLTC program, services, and enrollment process. Collaborate
with the Intake/Outreach and Clinical teams to coordinate timely,
appropriate onboarding of new members. Communicate professionally
with referral sources, physicians, caregivers, and community
partners. Participate in outreach activities to support community
engagement and referrals. Qualifications: Active New York State RN
license required. Bachelor’s degree in Nursing required. Minimum 1
year of acute care experience . Proficiency in UAS-NY assessments
required. Experience in home care and care management preferred.
Strong communication and interpersonal skills; fluency in Spanish
or Chinese is highly desirable. Proficiency in Microsoft Office
