Our commitment to quality and integrity has solidified our reputation as a trusted partner in the industry.*Role Description*:The Registered Nurse will be expected to have knowledge of discharge planning and make referrals. The RN will typically work on a Med-Surg or Tele unit and collaborate with a Social Worker. The nurse must be able to function independently and work as part of an interdisciplinary team, including physicians, for discharge planning. This position involves rounding with the team and contributing to discharge decisions for patients.*Caseload & Duties*:* *Caseload*: Typically between 17-25 patients per nurse, with the occasional lighter caseload on specific floors.* *Common tasks*: Assist with discharge to home, community, or SNF; coordinate housing or shelter, hemodialysis, wound care (including KCI referrals), and more.* *Experience*: Must have 1 year of inpatient case management experience.
