Their Assertive Community Treatment (ACT) program is designed to deliver personalized, community-based care to help individuals achieve stability and improve their quality of life. We are committed to a holistic approach that addresses both the physical and mental health needs of their clients.We are seeking a compassionate and dedicated Registered Nurse (RN) to join their Assertive Community Treatment (ACT) team. As an ACT Team RN, you will play a crucial role in providing integrated healthcare services to individuals with severe mental illness, working directly in the community to deliver care where its most needed. This position requires a commitment to person-centered care, flexibility, and a passion for working with vulnerable populations.*Key Responsibilities:** Direct Patient Care: Provide nursing care to clients in their homes or community settings, including medication administration, health assessments, and monitoring of physical health conditions.* Care Coordination: Collaborate with a multidisciplinary team, including psychiatrists, social workers, and peer specialists, to develop and implement individualized care plans.* Crisis Intervention: Respond to and manage psychiatric and medical crises in the community, providing immediate support and coordination of care as needed.* Health Education: Educate clients and their families about health conditions, medications, and healthy lifestyle choices to promote overall well-being.* Medication Management: Oversee medication administration, including injections, and monitor for side effects, adherence, and effectiveness.* Documentation: Maintain accurate and timely records of all nursing interventions, assessments, and communications in accordance with organizational policies and regulatory standards.* Community Engagement: Build strong, therapeutic relationships with clients, advocating for their needs and connecting them with additional community resources and services.*Requirements:** Associate or Bachelors degree in Nursing (ADN or BSN) from an accredited institution.* Licensure: Active and unrestricted Registered Nurse (RN) license in the state of NY.* Experience: Previous experience in mental health, community health, or a related field is strongly preferred.* Skills: Strong assessment and clinical skills, with the ability to work independently and as part of a team. Excellent communication and interpersonal skills, with a commitment to delivering compassionate care.* Flexibility: Willingness to work in various community settings and travel within the service area. Some evening or weekend hours may be required based on client needs._Please note- The annual base salary range is $80k to $95k. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things.
