Or a lot of RN experience in a Nursing home Expertise in primary care, acute care and chronic care practices Knowledge of health and patient care regulations Valid nurse practitioner license in NY Masters degree; PhD is a plus.As the nurse practitioner at our Skilled Nursing Facility, you will be at the forefront of providing top-quality care to our residents. In this role, you will work closely with our interdisciplinary team and physicians to ensure that all of our residents receive the highest level of care.Responsibilities include but are not limited to: Examine patients and their medical records when they are admitted to the Nursing home Creating an Advanced care plan Transitional care management (if they are coming from a hospital) Discharges Monthly check ups (Overseeing Chronic conditions) Diagnose health conditions and illnesses Propose treatments for chronic and infectious diseases Prescribe medications Maintain accurate records and schedules Inform patients about their health conditions and prescribed medications Oversee patients overall healthIf you're a skilled and compassionate nurse practitioner looking for a challenging and rewarding opportunity, we encourage you to apply!#NP3by Jobble
