Job Description:

All Metro Health Care, a Modivcare Personal Care Service, is looking for an RN Clinical Supervisor responsible for assessing, planning, and implementing safe, quality homecare services to clients. You will report to the Director of Clinical Services.You will --- Performs on-site supervisory and teaching visits to evaluate professional and paraprofessional staff on assigned cases and provides follow-up information to the Director of Clinical Services and physicians.--- Provides clinical orientation to employees on assigned home care cases.--- Counsels' caregivers in meeting clients' needs--- Performs initial nursing assessments as designated by the Director of Clinical Services and assists in the assignment of qualified personnel.--- Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient's condition/needs.--- Develops a multidisciplinary plan of care for implementation by utilizing the nursing process and modifying as necessary.--- Prepares clinical progress notes and reviews documentation continually, ensuring compliance with all Metro health Care documentation standards.--- Reports to the Director of Clinical Services and/or physician any changes in the patient's condition or needs.--- Attends case conferences, staff meetings, and in-service programs.--- Adheres to agency and patient care policies, including implementing patient rights and responsibilities.--- Monitors clinical records for complete and accurate documentation and adherence to policy.We are excited to speak to someone with the following --- Bachelor's degree as required by state regulations.--- Registered Nurse (RN)--- 2-3 years of supervisory or management experience required.--- 2-3 years experience as a Registered Nurse required, preferably in an acute care setting.--- Valid NYS Driver's License, insurance, registration, and reliable car--- Bilingual in English & Spanish requiredWe value our team members and realize the importance of benefits for you and your family.
