Job Description:
Job Summary Responsible for planning, tracking and implementing
process improvement projects to support the Population Health
program objectives.The Population Health Program Coordinator will
be responsible for helping Skagit Regional Health transform
healthcare by supporting our clinical teams. This will require
working towards improving quality, access, and reducing costs. This
role will involve working with practice managers and providers to
continue ongoing education which enhances workflows, communication
of best practices, and quality/ quality reporting, gap closures for
our goal of a healthier community. This position will coordinate
and communicate barriers to Director of Value Based Care for
ongoing improvement of organizational goals and strategies. The
person(s) in this position will coordinate and lead the projects
from start to finish. Essential Functions Plans, develops and
executes process improvement to support population health program
goals. Develops work plans, summaries, communicates as needed to
meet program objectives. Responsible for planning, tracking, and
managing all activities of the projects, including but not limited
to reviewing dashboards of quality metrics performance. Prepares
project status reports as needed to stakeholders and governance.
Serves as a liaison and educator between population health,
leadership, and other staff as needed to maintain the population
health program objectives Utilizes all resources available, uses
critical thinking skills and creative approaches to work through
obstacles. Learns, understands, and assists with workflows within
the EHR as necessary to meet program objectives. Consults with
analysts, trainers, clinical application specialists to understand
processes, data collection, report details, and other aspects
associated with population health program objectives. Maintains
project documentation including resources used, decisions made,
outcomes and agreements. Complies with SRH policies and procedures.
Performs other duties as assigned by Director or Designee.Promotes
a culture of safety. Education Bachelor's degree in health
informatics, business, public health, population health or related
field, required; Master's degree in Public Health, Health
Administration, Business Administration, or other related area,
preferred. Experience/Training At least 1 year of data analytic
experience, required. Previous experience in healthcare, highly
desired. Patient Centered Medical Home Model (PCMH) and care
coordination experience desired. Experience in the areas of
population health, quality improvement, project management or
process improvement, preferred. Electronic Medical Record, managing
reporting or equivalent skills, preferred. Strong skills in
Microsoft Office (Excel, Word, PowerPoint) are required.
License/Certification PMP certification (experience managing
projects), preferred Training/Certification in Lean or Six Sigma,
preferredClinical degree with current and unrestricted Washington
State license, preferred. Other Qualifications Strong verbal,
written and interpersonal communication skills. Demonstrated
organizational skills. Ability to identify and resolve problems or
difficulties. Strong critical thinking skills. High degree of
self-motivation and professionalism with the able to take
initiative. Ability to use qualitative and quantitative data.
Analytical mindset that understands and can explain data to others
effectively. Ability to maintain a calm and professional demeanor
and elicit cooperation across the organization. Proficiency with
Microsoft Office products, including but not limited to working
with spreadsheets, reports and databases. An ability to work under
deadlines in a highly demanding environment. The candidate will
need to have excellent interpersonal skills to be able to work with
multiple groups of people with varied skill sets, as well as in a
team environment.
