Job Description:

Job Title: Care Management Team LeaderLocation: Mt Vernon, NYCare Management Team Leader (CM TL) will co-lead the team under the direct supervision of the CM Coordinator (and the Director). They will support and motivate team members, monitor performance, and provide effective guidance. The CM TL must be able to act proactively to ensure smooth team operations and effective collaboration.
